Later, this Licensing Specialist observed a child exit the same classroom, walk down the hallway, and enter the kitchen, unsupervised. Retrain Staff #5 on proper supervision of children.

This was corrected at the visit when the new staff person left the center.

At 12:06 pm Staff #7 left the classroom to check on child #4 to see if they used the bathroom leaving seven children alone in the classroom. Ensure while staff are providing direct care, staff is not responsible for other duties. Staff #7 answered the door and then called on her personal cell phone to get the director to come from upstairs. This was discussed and corrected with staff, and they agreed to comply.
