Ensure a working thermometer is used to ensure refrigerators are at 41 degrees F or colder. The thermometer in the refrigerator located in the infant classroom read 49 degrees F. Ensure prepared food is labeled with contents and the date of preparation. Leftover containers were observed in the kitchen refrigerator. The containers were not labeled.
Toilet Facilities (regulation 51U)
Ensure toilet paper and paper towels are accessible in toilet facilities. Toilet paper was needed in the women's bathroom and paper towels were needed in the men's bathroom. This was corrected when the items were placed in the bathrooms. Ensure bathrooms are kept clean. Clean the men's bathroom.

Ensure equipment in the outdoor play area is in a safe condition. Remove the broken net from the outdoor play area. Ensure the gate to the outdoor play area is self-closing and self-latching. The gate closet to the shed is not self-latching.

When asked, staff stated the cots are only cleaned weekly. Ensure safe sleep practices are used. The fitted sheets on two cribs were observed to be not tightly fitted to the crib mattresses.

Ensure each classroom is supplied with sufficient quantity of developmentally-appropriate materials. The pre-k classroom needs manipulatives/math/problem solving. science, and music materials. The three-year-old classroom needs dramatic play and science materials. The two-year-old classroom needs language/literacy, manipulatives/math/problem solving, dramatic play, and science materials.
Diapering and Soiled Clothing (regulation 61U)
Ensure a foot-activated trash can is used solely for diapers. A foot-activated trash can was observed in the women's bathroom labeled "wet diapers and trash." The trash can's lid was observed on the ground behind the trash can. Diapers and regular trash were observed in the trash can. This was discussed and corrected when the labels were corrected and the bathroom was supplied with a foot-activated trash can for diapers only and a covered trash can for trash. Ensure a complete Medication Administration Record (MAR) is obtained for all medication. Four children need complete MAR's on file. Ensure medication is labeled with children's names and stored out of children's reach. No diaper cream was labeled. A can of Aquaphor was observed on the teacher desk in the two-year-old classroom and was accessible to children. This was corrected when the Aquaphor was moved. A can of corn starch was observed on the changing table's shelf. The corn starch was accessible to children. When asked, staff stated it is used on children when they have a diaper rash. No written parent permission was observed on file for the corn starch. This was discussed and corrected

Ensure infants are held for bottle feeding when too young to use a feeding chair. A staff member was observed holding an infant's bottle and feeding them while the infant was seated on the floor in a boppy seat. This was discussed and corrected when staff member picked up the infant to feed them. Ensure bottles are warmed in water no more than 120 degrees F. When asked, a staff member stated she uses a microwave to warm the infant's bottles. This was discussed and corrected when the staff member agreed to use hot water in the future. Ensure bottles are labeled. No bottles in the infant classroom were labeled.
